2. Select the type of invitation you received to enter the survey, either a survey invitation card or sales receipt.
3. If you have received a survey invitation card, enter your 10-digital number located at the bottom of your card, and then click on the button marked “ENTER” to get started. If you are sales receipt participant, enter the store telephone number, patient number, your visit date and time, and then click on the button marked “ENTER”.
4. Follow the on screen instructions to answer all the questions and submit your survey.
5. Once completed the survey, you will be asked to enter your personal information to take part in the sweepstakes to win £1,000 or an iPod instant prize.
