It helps to make a number of commercial activities for businesses in Florida easier and faster, such as various business entity filings, trade and service mark registrations, federal lien recordings, judgment lien filings, uniform commercial code financing statements, fictitious name registrations, notary commissions, and cable and video service franchises.

  2. Click on the button at the bottom of the web page marked “Customer Satisfaction Survey”.
  3. Select the method you used to contact the Department of State and then click on the button marked “Submit”.
  4. Answer the survey questions and then click on the button marked “Submit”.
  5. Follow instructions to complete the survey.
