- Go to the Pep Boys Online Survey website at Resource below.
- Choose between “SERVICE REPAIR” and “PRODUCT PURCHASE”, depending on what item you purchased.
- Follow instructions to enter your personal information.
- For “SERVICE REPAIR”, enter the Work Order # printed on you receipt. For “PRODUCT PURCHASE”, enter the Transaction # and Store # instead.
- Click on the button marked “Click Here to Start Survey”.
- Answer the survey questions based on your experience at Pep Boys.
- Once completed, you can enter the sweepstakes. The $5,000 might be yours. Good luck!
