- If you would like to take this survey in Spanish, please click on the link marked, “Español(PR).”
- Enter the 16-digit survey pin number as it is printed on your PetSmart sales receipt and then indicate which PetSmart website address is printed on your sales receipt. Next, click on the button marked, “Start.”
- Answer the survey questions as directed.
- After you complete the survey, you will be given a validation code that you may write down on your PetSmart sales receipt. Take this receipt to PetSmart to redeem the offer that is printed on it.
