- Tick the circle to indicate the language in which you would like to take the survey, English or Spanish, and then click on the button marked, “Continue.”
- Select the year of your birth from the drop-down menu and then click on the button marked, “Continue.”
- Indicate whether you or anyone in your family is employed by Sam’s Club and then click on the button marked, “Continue.”
- Read the survey instruction and then click on the button marked, “Continue.”
- Enter the Club Number, TC Number and date of your visit to a Sam’s Club as it appears on your sales receipt. Then, click on the button marked, “Continue.”
- Complete the survey and enter the sweepstakes as directed.
